For pain, participants treated in multiple visits were less likely to be in pain the week after starting treatment when compared to participants treated at a single visit (evidence from 5 studies). But for other outcomes, such as the presence or absence of pain after obturation and its intensity, use of analgesics, swelling, and the presence or absence of a sinus tract, there did not appear to be appreciable differences. Despite the variability and limitations of the studies, the research team remains moderately confident in the finding that pain in the first week after treatment is more likely with a single-visit root canal. “There is still a valid place for traditional multiplevisit endodontic treatment,” says Dr. Dabuleanu. “Multiple visits allow for placement of an intracanal dressing and calcium hydroxide remains the dressing of choice in situations of questionable endodontic prognosis, such as teeth presenting with large lesions or deep periodontal pockets.” Multiple visit endodontic treatment is also the protocol of choice when dealing with acute infections where drainage occurs through the canal system. Multiple appointments allow for acute infection resolution, healing of endodontic sinus tracts and improvement of patient symptoms.
